Connections between Hospital & Rehabilitation Discharge Planning and Accessible Transportation Options
Easter Seals Project ACTION (ESPA) wants to understand and foster the connection between discharge planning and patients’ or clients’ knowledge about, understanding of, and access to accessible transportation options. Transportation connections, particularly for people with disabilities and older adults, provide a means to independence and inclusion in community living. Access to desired destinations—doctor’s appointments, grocery stores, shopping centers, homes of family and friends, and places of worship—improves a person’s quality of life and is linked to good health, longevity, and lower medical costs. AARP has documented a correlation between access to social activities and healthier, happier living for older adults. In addition, community-based healthcare offers personal and societal cost-savings compared to institutional care.
